The Rural Wellington Addiction Services Team works in partnership with the Upper Grand Family Health Team to provide low-barrier support to individuals, friends and family, and service providers identifying concerns with substance use. Services are provided by an addictions counsellor and/or peer support worker.
Program Structure
Services include assessment, treatment/care planning and counselling and support services. The Rural Wellington Addiction Services Team will also assist clients in accessing other community services based on their identified needs and goals.
Family & Friends Support
The Rural Wellington Addiction Services Team can provide support and education to the family and friends of individuals living with substance use concerns.
Provider Consultation
The Rural Wellington Addiction Services Team can provide consultation and education support for primary care providers and community workers supporting individuals with substance use concerns.
